How to Order PDHM?

 

The Milk Bank provides pasteurized donor human milk (PDHM) by prescription or physician order to hospitals and outpatients throughout the Midwest and United States. We believe all babies deserve human milk, especially premature and ill infants in hospital neonatal intensive care units.

Complete Outpatient Intake Forms

Complete brief outpatient forms and obtain a prescription from your baby’s health care provider with the following information:

  • Pasteurized donor human milk
  • Detailing the amount of PDHM prescribed in ounces per day
  • How long the prescription is valid (1 week, 1 month, etc) with a max of 3 months
 

 

Payment

Make payment arrangements with The Milk Bank. We accept Visa, MasterCard, check, or you can make payment plan arrangements. Please call us for more information on cost and payment.

 

 

Insurance

Insurance coverage cannot be guaranteed. We encourage you to submit claims for reimbursement through your private insurance company. We will do our best to provide the information you may need. However, at this time, we cannot directly bill insurance companies for PDHM on an outpatient basis.
